The following documents are provided as a courtesy of our Global Ministries department.  All documents are required and must be completed for Northside Baptist's short term missions trips. 

 

These documents are available to any ministry who wishes to use these as they are or as a model of what they want to design for their own needs. If there are any questions, please contact the Global Ministries Office at Northside Baptist Church.
 

 

 

#1 PRIORITY -- PASSPORT

 

If you already have one, please check the expiration date. The expiration date must be effective at least six months from the date you are scheduled to RETURN from your trip.  If you do not have a passport, do it TODAY! It can take longer than you might think to obtain a US Passport.
